Consumer Directed Services — CDS

Hire a Family Member or Friend as an Attendant

Heart and Home Care can help you enroll in the CDS program (Consumer Directed Services). A friend or family member may be hired as an Attendant, except for a spouse or anyone under 18.

With CDS, participants direct their services and choices. This program is designed for consumers with disabilities and seniors to live independently in their homes.

To qualify for CDS services, a person must:

  • Be receiving or eligible to receive Medicaid Benefits
  • At least 18 years of age
  • Have a physical disability.
  • Be able and willing to recruit, hire, and manage their caregiver attendants.
  • Be able to self-direct their care services.

Before being employed by a CDS participant, the attendant must have a clean background check through the MO State Family Safety Registry.

What Kind of Help Can I Receive at Home?

Through a State approved Plan of Care, an individual can have assistance with daily tasks such as:

  • Meal preparation
  • Medication reminders  Personal Care
  • Light household tasks
  • Laundry
  • Transportation for shopping & errands

How It Works

To qualify for caregiver services, the individual receiving Medicaid benefits must meet the Missouri Division of Senior and Disability Services (DSDS) requirements.

DSDS will complete an assessment to determine the individual's needs and forward a personalized care plan to Heart and Home Care, LLC. Once a care plan is received, Heart and Home Care can help you get started! We handle all the paperwork at no cost to you.
